Validation of Simple Nutrition Screening Tool (SNST) as a screening tool for hospital malnutrition in Indonesia

Background.

Malnutrition in hospitalized patients is considered as a significant problem, thus nutritional screening should be performed in all patient on hospital admission.In Indonesia was developed nutrition screening tool, namely the Simple Nutrition Screening Tool (SNST) which was valid and reliable. The objective of this study was to compare the SNST and Nutrition risk screening-2002 (NRS-2002) against the Subjective Global Assessment in patients admitted to Sleman District Hospital.

Methods. This was an observational study to compare assessment of nutritional status by SGA with nutrition screening using the SNST and NRS-2002. Two hundred and eighty six adult patients admitted in surgical and internal ward were included as research subjects. Contingency table and ROC curve were used for measuring validity of each screening tools.

Results. The SGA tool identified 100 (35 %) patients as malnourished and 186 (65 %) patients as well-nourished. The SNST and NRS-2002 identified nutritional risk in 44% and 36% of the patients. The SNST had a sensitivity of 99 % and a specificity of 85 %, and the NRS-2002 had a sensitivity of 84 % and a specificity of 89 %. The NRS-2002 had higher positive predictive value (81%) than the SNST (78%). Otherwise, The SNSThad higher negative predictive value (99%) than the NRS-2002 (91%). Predictive value (ROC AUC) of both screening tools was outstanding at 0.96 and 0.93 for SNST and NRS-2002, respectively.

Conclusions.The SNST had an excellent sensitivity for identifying patients who were malnourished or at risk of malnutrition in the inpatient setting. The SNST can measure nutritional status equally well compared to NRS-2002.
